Annual SELF Underwriting Questionnaire

Adobe Reader does not allow saving, so how can the underwriting questionnaire be saved?
The underwriting form is an interactive, fillable form containing your district's information. Fields embedded in the form store data securely in an encrypted format, not hte Adobe Reader Program.
 

Can the questionnaire keep last year's data to eliminate the need to re-key information?

Definitely! All districts that have submitted the form in the past will receive a copy that is populated with the information from the previous year. If you receive a form without data, that means we did not receive an underwriting submission from you the previous year.

 

I was concerned about losing my data and do not have a saved copy of my form.

When you choose "Save copy via E-Mail" your data is saved as an attachment and e-mailed to the recipient of choice. Also, at the time of submission, you can check "Yes" in the last check box on the forma dn you will receive a copy for your records and a confirmation of receipt from Schools Excess Liability Fund.

 

Can I send the questionnaire to other departments in order to get it completed?

Select the "Save copy via E-Mail" option and add the e-mail of the person you wish to send the form to. It can be opened by any user with Adobe Reader.

 

Our internal security did not allow us to download and save our information.

The underwriting questionnaire will generate an e-mail wiht a file name tmp3483.pdf, for e.g., wiht a subject of "Saved Copy of Liability Underwriting Renewal Data Form" and will e-mail it back to the person filling out the questionnaire for your district. Various districts with spam filters and e-mail blockers may internally block this attachment, so your IT Department will need to add the @selfjpa.org address to your e-mail white list.
