Step 1. - In Macintosh, after you have begun to update your underwriting questionnaire and wish to save, navigate to the last page of the survey and use "Save Form" button on the lower left. The Adobe Reader Extensions will securely transmit your data and begin the saving process.

Step 2. - By Choosing the "Save Form" at the bottom left on the last page, the prompt will appear "Unable to establish a secure connection to "www.formrouter.net". Data typed into your questionnaire is stored in this secure location and uploaded back to your form when you re-open to continue adding data. You will also get this message at the time of electronic submittal to Schools Excess Liability Fund. Please choose Continue.

Step 3. - By choosing continue, the PDF is ready to download prompt will be generated by the questionnaire. Your data entered in the form is being transmitted back to your workstation. In Macintosh the green download button will not appear during this process. Please choose "OK" and Continue.

Step 4. - To save your form to your desktop, use the Save Icon in Adobe.

Step 5. -Adobe Reader will issue a prompt showing the "document will not allow you to save" next. The Schools Excess Liability Form's Reader Extensions are engineered to work in Mac OS 9.1 with Reader versions 5.1 and above, allowing districts to complete and save in the location of your choice.

Step 6. - By choosing "OK" the saving your form process will ask where you would like the questionnare stored during the closing of Adobe. Your districts data has been safely transmitted and stored in a secure location. It is recommended to save on your Desktop, or local drive. The document can be sent via email or copied to another destination if multiple departments are involved in the questionnaire process. The default name of the form is shown below. Choose "Save" .

Step 7. - After choosing "Save" the next prompt is to overwrite your changes to the form called "processform.aspx". This is the default name of the Schools Excess Liability Fund Questionnaire. Choose "Replace" to update your data before you submit your districts information. On submit and the e-mail address listed will receive confirmation that your districts information has been received.
