Step 1. - The first prompt gives a Security Warning. Adobe Reader 7.0 is identifying the "Reader Extensions" Schools Excess Liability Fund has embedded in this questionnaire to allow districts to save data. SELF is storing your data in a secure site with each save. This is a generic message generated by Adobe Reader, not the SELF Questionnaire. Simply click Yes and continue.

Step 2. - By Choosing the "Save Form" at the bottom left on the last page, the prompt will appear to download the form to your workstation. Click OK and continue.

Step 3. - After choosing the "Save Form" button and it turns green with the "Download" message, this prompt will appear showing Adobe Reader cannot save form information. The code within the form wil safely and securely store your data. Please choose Close and continue.

Step 4. - By clicking download, the "Save a Blank Copy of this Form dialog box will appear (another Adobe Reader 7.0 prompt). Choose the Save a Blank Copy Box and continue.

Step 5. -When your form is ready to download, the save a copy box will appear. The default name for the form is processform.pdf which can be saved in the location of your choice. It is recommended to save on your Desktop, My Documents or local drive. The document can be sent via email or copied to another destination if multiple departments are involved in the questionnaire process.

Step 6. - After retreiving your form and continuing to fill out the questionnaire, Adobe Reader will issue this prompt at the time of a save and closing of Adobe. The "Form Data Not Saved" will appear. Your districts data has been safely transmitted and stored in a secure location. To exit this prompt, choose the "Discard Data and Close Form" as this refers to normal Adobe documents, not the Schools Excess Liability Fund forms engineered to save in Adobe Reader versions from Reader 5.1 to 7.0.5.

Step 7. - When the questionnaire is complete, and the "Submit Questionnaire" button is pressed, Adobe Reader will issue the same prompt as in Step 3. (Cannot Save Information), after choosing Close, the last of the prompts will appear. The Security Warning shows the secure site Schools Excess Liability Fund has stored your districts data. If you wish to print a copy of your survey, choose "Block" if you are finished and ready to submit, choose "Allow" and your data will be submitted and the e-mail address listed will receive confirmation that your districts information has been received.
