1. Open the link and access the underwriting questionnaire.

2. Once you open the form, please complete the form with your district's information.

3. Should additional information need to be entered that is not readily accessible, you are able to save your current changes and open the form again once you have the information.

Choose the "Save Form" tab located at the bottom of the last page of the questionnaire.

   A prompt will appear indicating your PDF form is ready to download. Press "OK."

   Scroll to the bottom of the form. Find and select the green "Download" button.

   You may now save the form to the local directory of your choice.

   To re-open your form, simply return to the saved location in your directory.
     


4. Once the form has been updated, please review the changes and choose the "Submit Questionnaire" tab located at the bottom of the last page of the form.

5. You will receive confirmation that the form has been received by SELF.

6. If you have any questions, please review the on-line help for the version of Adobe Reader you are currently using. Illustrations are shown for both Adobe Reader 6.0 and Adobe Reader 7.0 versions with the prompts and saving features of the Schools Excess Liability Fund reader extensions as districts save their data into the underwriting questionnaires. All forms have been engineered to save in Adobe Reader versions 5.1 through 7.0.5

7. If after you have submitted your questionnaire and need to change any data, please e-mail Melissa Smith or call her at the SELF Office at (866) 453-5300. Any technical questions/difficulties with the form should be directed to Alan Grant.

8. If your district uses Macintosh and are experiencing difficulties with the form, please review the Macintosh Help Viewer, The forms for Mac have been tested to save data, with java code enabled and submit securely in Mac OS 9.1 with Adobe Reader 5.1 and above.

9. If you prefer to fill out your questionnaires in a Microsoft Word document, please choose either the Excess Liability form or the Excess Workers Compensation and fax to the SELF office, Attention: Alan Grant at (916) 321-5311 by March 31, 2006.
