1. Open the link and access the underwriting questionnaire.

2. Once you open the form, please complete the form with your district's information.

3. Should additional information need to be entered that is not readily accessible, you are able to save your current changes and open the form again once you have the information.

Scroll to the bottom of the form and choose "Save Form via E-mail " when you are ready to save.

   Input e-mail address you would like a saved copy of your renewal form sent to.
     
C.
   The Adobe form will generate a .pdf attachment and e-mail
     


4. Once the form has been updated, please review the changes and choose the "Submit Questionnaire" tab located at the bottom of the last page of the form.

5. You will receive confirmation that the form has been received by SELF.

6. If you have any questions, please review the on-line help for Adobe Reader.

7. If after you have submitted your questionnaire and need to change any data, please e-mail Melissa Smith or call her at the SELF Office at (866) 453-5300. Any technical questions/difficulties with the form should be directed to Alan Grant.

8. If your district uses Macintosh please use this Macintosh link. The forms for Mac have been tested to save data, with java code enabled and submit securely in Mac OS 9.1 with Adobe Reader 7.0 and Safari Browser.
