Data Collection

Annually, SELF collects claims information from members for the liability actuarial study. The studies provide the Board with valuable trending information and funding options. Each SELF member is required to provide loss reporting information to SELF. The loss information provides the actuary with a complete picture of SELF's exposures from which to determine future rates.

Request data from GL members:
Members Attaching at $1M = June 29
Members Attaching at $5M = July 1

Deadline for GL data submission to SELF:
Members Attaching at $1M = July 15
Members Attaching at $5M = July 31
*Members who do not submit COMPLETE AND USABLE data to SELF by the deadline above will be charged 20% of their current year's invoice unless prior arrangements have been made with SELF staff. ​

SELF Underwriting Renewal Forms
Access links to the Annual Underwriting Renewal Forms for the Excess Liability Program will be sent via e-mail August 1 with a due date of August 29.
The link will be sent to your district/agency’s representative, along with the credentials to log into the form and access your prior year information. Please update any information that has changed, answer any new questions and submit your completed form by the deadline. If this is the first year you are receiving this form you will need to fill it out in its entirety.
